Personal Information Protection Policy

Sakura Community Service Co., Ltd. conducts business activities via "Living Support for The Elderly", "Nursing Care Training" And "Employment Support". Personal information handled through these business activities is very important to us and we recognize that it is our company's important social responsibility to securely protect your personal information.
Therefore, we will establish the following personal information protection policy and fulfill our social responsibilities on personal information protection. Hence, in the "Living Support for The Elderly" business, we comply with laws and regulations of the Ministry of Health, Labor and Welfare's "guidelines for proper handling of personal information by medical and nursing-care business provider", and, based on the philosophy of respect for individual and the basics of social ethics, we will work on proper handling of personal information.

1. We limit the usage to the extent which is necessary for business activities, clearly define the purpose of usage; obtain, use and provide personal information appropriately. We will only use the acquired personal information within the scope of the purpose of use and use protection methods to prevent out-purpose usage.
2. We will comply with laws, guidelines and other regulations regarding the handling of personal information established by the government.
3. In order to prevent leakage, loss or damage of personal information held by the Company, we will take reasonable methods and take necessary precautions.
4. We will set up complaints and consultation desks concerning handling of personal information and efforts regarding protection of personal information.

With respect to the disclosed personal information held by the Company from the person himself or his representative, (1) Request for disclosure,

The procedure of our correspondence is as follows. "Disclosed Personal Information" means "A collection of information structured systematically so that

rules with organizing, classifying, table of contents, index, sign etc., specific personal information, from which to create a collection of systematically
structured information, can be easily retrieved. The Company has the authority to respond to all requests from the person himself regarding the
disclosures, corrections, additions or deletions of contents, suspension or elimination of use, suspension of provision to third parties”

・About request for disclosure
For requests for disclosure, please attach the necessary documents regarding the personal information disclosure request etc., and mail to the following "Contact Information”. When sending the documents to us, please do it in a way that you can check the delivery record, such as delivery record mail or simple registered mail. In addition, we would greatly appreciate if you could write "enclosed personal information request" in red in the envelope.

・Request for disclosure by representative / proxy
When delegating to a proxy for requesting disclosure, please enclose the following documents in addition to the request for personal information disclosure
(1) Documents for confirming that you are the representative (copy)

(2)Power of attorney After the person himself sealed in the power of attorney, please attach the stamp registration certificate of that seal. When an agent is a legal representative, such as parental authority, instead of the power of attorney, it is also possible to submit the documents explaining the relationship between the principal and the related person. In the case of request for disclosure etc. concerning nursing care business, we will sufficiently explain the representative and the details of the disclosure target personal information to the principal, confirm the individual's intent, and judge the appropriateness of the request of the representative. After verifying the intention of the individual about the scope of disclosure, the principal stamps on the power of attorney and attach the stamp registration certificate of that seal.

・Fee related to request for disclosure and notice of purpose of use
In the case of requesting disclosure of personal information and notice of purpose of use, a fee of 500 yen (including tax) will be charged for each request. Please enclose 500 yen stamps in the submitted envelope. Customers will be responsible for shipping costs to our company. Please note that we will not be able to notify you of the disclosure and usage purpose when the fee is insufficient or the fee is not enclosed in the envelope.
・Method of respond to requests for disclosure
We will reply in writing to the address stated on the invoice of the claimant.

3.About the handling of credit card information of customers using credit card
The purpose for which we acquire credit information (card name, card number, validity period) from the customers, Name of the acquirer of the credit information, Name of the information provider, and the retention period are as follows.

For transaction for your purchasing of goods and services by credit card.
